We need to understand why we failed – because i) sand was not available? ii) cement was not available? or iii) resources were not available? iv) workers were not available? or v) equipment was not available? If all were available, then was it because design was incorrect? Then we need to find out was it one off or a systematic failure? or was it out of our control?
